Parkinson's Disease Specialist Nurse
This role provides comprehensive support for patients, carers, and families living with Parkinson's disease in a designated region.
1. Manage patient caseloads, conduct needs assessments, develop care plans, implement and document interventions, and evaluate outcomes.
2. Provide specialized support services to service users, prioritizing the needs of newly diagnosed individuals with PD.
3. Foster therapeutic relationships by actively engaging patients and families in their treatment.
Key responsibilities include delivering direct and indirect care, serving as an expert resource, offering guidance on diagnosis and management of PD, and establishing protocols for best practice.
