Purpose of the Post:
The HSE Dublin and Midlands serves a population of approximately 1,077,639, providing health and social care services across Dublin West, Dublin South-West, Dublin South City, Laois, Longford, Offaly, Kildare, West Wicklow, and Westmeath. About 30,000 staff work within this region with a budget of €3 billion.
The region comprises 4 Integrated Health Areas (IHAs), each serving around 300,000 people, combining acute and community services, including hospitals, care hubs, and community health networks. These IHAs facilitate integrated care delivery, bringing together various health services and providers.
The reform programme aims to shape future health and social care services through regional service coordination and delivery, enabling effective planning and management across communities.
Older Persons Service Model:
This model focuses on managing frail older adults with complex needs, aiming to reduce hospital admissions by providing timely diagnostics and specialist services, and supporting patients in the community post-discharge.
Integrated Care Programme for Older Persons (ICPOP) Community Specialist Team (CST):
This transdisciplinary team manages complex care needs of older adults with multiple co-morbidities, supporting a person-centred approach, reducing emergency visits, and providing education and support to patients, carers, and healthcare professionals.
Clinicians will collaborate with geriatricians and the ICPOP team to develop care pathways and work across primary, community, and acute care services.
Role of HSCPs:
Health & Social Care Professionals (HSCPs) are regulated professionals committed to high standards of care, supported by CORU. Care Assistants support HSCPs in delivering quality care to ensure optimal patient outcomes.
