We currently have an opening for an experienced Home Health Care Registered Nurse. RN candidates with ICU, PICC Line, Wound Care, and Infusion experience are highly preferred.
The corporate office is located in Southfield, MI. The individual will provide RN support throughout our service area within the Tri-County area (Oakland, Macomb & Wayne).
Job Overview:
A Registered Nurse administers skilled nursing care to patients in their homes, following physician orders and care plans, under the supervision of the Clinical Manager/Nursing Supervisor.
QUALIFICATIONS:
* Graduate of an approved nursing program and currently licensed in Michigan
* At least one year of nursing experience
* Kinnser - EMR experience preferred
* Home Healthcare experience preferred
* Experience with wound vac, PICC line, mediport care preferred
* Pediatric experience is a plus
RESPONSIBILITIES:
* Provide services according to the plan of care.
* Conduct initial and ongoing patient evaluations.
* Develop and revise care plans as needed.
* Perform specialized nursing procedures.
* Initiate preventive and rehabilitative nursing interventions.
* Document patient visits and care summaries promptly.
* Coordinate services and communicate patient condition changes.
* Counsel patients and families on care needs.
* Participate in in-service training and team meetings.
* Follow agency policies and procedures.
* Manage orders, notify physicians, and complete required documentation.
* Assess and determine nursing needs for each patient.
* Refer patients to other healthcare professionals as needed.
* Supervise and educate other nursing staff.
* Lead patient care conferences.
* Participate in peer review and performance improvement initiatives.
* Assist with utilization reviews.
* Provide total patient care when necessary.
* Participate in on-call duties during nights, weekends, and holidays.
* Complete OASIS assessments and ICD-10 coding accurately and timely.
WORKING ENVIRONMENT:
Work involves indoor office settings, patient homes, and travel between locations.
