Community Staff Nurse (General, Disability & Mental Health) Location: Duleek, County Meath Salary: €56,000 - €60,000 per annum Reporting Relationships Key Working Relationships Consultant Psychiatrists, General Practitioners, MDT, Director of Services and Assistant Director of Services Line Manager Operations Nominated Assistant Director of Services Professional Reporting Relationship Nominated Assistant Director of Services Purpose of Post: The overall objective of the position of Community Staff Nurse (Adult & Children Services) is to manage and provide clinical/holistic care to the highest professional standards, which meets the practical, physical and emotional needs of service users, with an intellectual disability/Acquired Brain Injury, and a co-morbid mental health diagnosis and often accompanying physical disabilities/sensory impairments, complex medical and healthcare needs. This care provided will be consistent with the mission, vision, values and strategic plan of the Talbot Group. Respecting, valuing and fostering the service users personal development, quality of life and abilities is central to the role of Community Staff Nurse. Essential Qualifications and Experience Be registered in the relevant Division of the Live Register of Nurses maintained by Nursing and Midwifery Board of Ireland or be entitled to be so registered. AND Have an in-depth knowledge of HIQA standards and the regulations underpinning the standards as they apply to the Intellectual Disability Services. AND Have the clinical, and administrative ability to properly discharge the functions of the role. Desirable Experience Have at least five years post registration nursing experience of which two must be in one of the specialist areas or related area. Professional Knowledge & Experience Experience of working with people with an intellectual disability and Acquired Brain Injury. Knowledge of mental health diagnosis and management Knowledge of the process of Person-Centred Planning. Knowledge of positive behaviour support framework Knowledge of quality assurance practices and their application to nursing procedures. To have working knowledge of National Policies, including De-congregation, Assisted Decision Making and other polices relevant to the service. To have a working knowledge of the Talbot Group policies. Legislation, Ethics and Regulation Demonstrates acceptance of responsibility that his/her nursing practice meets all required standards. Principal Duties and Responsibilities: The Nurse will be responsible for the following, ensuring privacy, dignity and respect of the service user at all times. Professional Provide a high level of professional and clinical leadership. Liaise with General Practitioners in relation to service users needs Manage service users health care needs, to ensure the highest professional standards using an evidence-based approach to ensure clinical effectiveness and efficiency. Develop risk assessments as needed. Develop Health Check Procedures & Protocols to support service users health & well-being. Develop pathways of onward referral to MDT colleagues as required. Coordinate and prioritise appointments in liaison with MDT. Monitor the quality-of-service delivery and provide professional advice and guidance to staff. Ensure an ZER0 tolerance approach to abuse. Assist in the development and implementation of clinical policies and guidelines which supports person centred services in line with National Quality Standards. Report and escalate any concerns in a timely manner Process any complaints in Line with Talbot group Policy Participate in quality improvement initiatives to improve service user satisfaction. To prioritise and respond to demands under pressure or in emergencies Maintaining a high standard of work performance, good attendance, appearance and punctuality at all times Carrying out any duties appropriate to the role which Director/Assistant of Services may determine from time to time, To always present and act in a professional manner. To ensure resident confidentiality is respected and always maintained. To be familiar with and act according to the Nursing and Midwifery Board of Ireland professional code of conduct. To participate in the identification, development and delivery of education, training, and development programmes for non-nursing staff on the management of service users diagnosis. To act as a role model to other staff and motivate them to higher performance. To communicate complex and sensitive information effectively to the service user, their families, and staff, overcoming any barriers to understanding and providing support during distressing or emotional events To maintain own competence in all areas of role. Person Centred Planning Initiate, manage, and promote the concept of person-centred planning enabling the service user to identify their personal goals. Ensure co - ordinated multi-disciplinary service delivery by working in partnership with MDT colleagues, staff, families and other key people to support the service user to work towards and achieve their personal goals, guided by individual care plan. To assess service users in a regular and systematic basis through the completion of suitable assessment models. Attend and participate in needs assessment, with and for service users in planning, implementation and evaluation of the care of service users Health and Safety Ensure compliance with the Code of Professional Conduct as laid down by Nursing and Midwifery Board of Ireland. Ensure safe practice in relation to medication as governed by Nursing and Midwifery Board of Ireland and Talbot Group policies and guidelines. To work with management in supporting non-nursing staff to become competent Authorised Medication Administration Persons by mentoring, supervising and completion and sign-off of competency assessment processes including ongoing professional oversight of non-nursing staff in the safe administration of medicines. Perform duties in a manner conducive to health and safety and in accordance with Talbot Group policies and procedures. To work with staff /PIC/ Assistant Director of Services in identifying risks to service users. Ensure that accidents and incidents involving service users/visitors/staff are recorded and investigated in accordance with policy. Training and Ongoing Development Be aware of current developments in the nursing profession and maintain a commitment to ongoing professional development. Promote and participate in nursing research/evidence based clinical audit and encourage its practical application to service user care within the service area. Attend and participate in education and in service programmes as required by the needs of the service user and in compliance with Talbot Group policies and national standards e.g., prevention, detection and response to abuse.
