Purpose of the role:
To provide comprehensive social work services as part of a multidisciplinary team, in line with principles of patient centred care that reflect the needs of the patients and good practice in social work as well as Bartra Healthcare standards.
Responsibilities:
* To provide a comprehensive social work and counselling service to all patients and their families.
* To demonstrate an excellent skill set in practice of communication and, interpersonal skills that effect strong co-working multi-disciplinary team professional relationships that foster person centred patient care collaboratively.
* To develop robust and comprehensive patient care plans in partnership with the patient, their family and the multi-disciplinary team to effect detailed and safe transfer/discharge.
* To demonstrate in practice the unique sociological model of care that social work provides in informing, educating and supporting colleagues understand the social model of care for patients and how it works in partnership with the medical model.
* To provide a comprehensive social work and counselling service to all patients and their families of the Beaumont Lodge,
* To develop strong co-working multi-disciplinary professional relationships that foster person centre patient care collaboratively.
* Maintain throughout an awareness of the primacy of the patient in relation to all activities.
* The post holder will uphold human rights, dignity and integrity for the patient, understanding and engaging in patient consent led support and intervention.
* To develop robust and comprehensive patient care plans/discharge plans in partnership with the patient, their family and the multi-disciplinary team to effect detailed and safe transfer to the community setting.
* Carry a caseload that incorporates for example cases that are a mix of complex, urgent, emergency, priority, routine, generic, episodic, and acute and many more.
* Performance management systems are part of the role and you will be required to participate in the Group's performance management programme and this will be undertaken on a regular and routine basis as directed by the Director of Clinical Services and the Principal Social Worker.
* The post holder will practice within CORU Regulatory Body Code of Ethics and Standards and Hospital regulations including adhering to Health and Safety regulations e.g. Covid-19 regulations and guidelines amongst other.
Risk Management, Infection Control, Hygiene Services and Health & Safety
* The post holder will report to the Director of Clinical Services. The management of Risk, Infection Control, Hygiene Services and Health & Safety is the responsibility of everyone and will be achieved within a progressive, honest and open environment.
* The post holder must be familiar with the necessary education, training and support to enable them to meet this responsibility.
KPI's
* The post holder will engage in the identification and development of Key Performance Indicators (KPIs) which are congruent with the Hospital's service plan targets.
* The development of Action Plans to address KPI targets.
* The development of presentations to illustrate trajectories.
* The development of the collation of weekly, monthly and or annual statistics as required
Experience and Qualifications:
Essential:
* Relevant 3rdLevel Qualification in Social Work
* CORU Registration
Desirable:
Relevant Postgraduate Qualification.
