Responsibilities:
1. Provide high-quality person-centred care to individuals with dementia or other health conditions.
2. Work collaboratively with healthcare professionals, family members, and other caregivers to ensure seamless care delivery.
3. Develop and implement individualised care plans that meet the unique needs and goals of each client.
4. Conduct regular assessments to monitor client progress and adjust care plans as needed.
5. Communicate effectively with clients, families, and colleagues to ensure smooth communication and teamwork.

Required Skills and Qualifications:
Qualifications:
FETAC level 5 qualification is desirable but not essential.
Skills:
Good communication and organisational skills,
Ability to work independently and as part of a team,
