Job Description:
The role of a Clinical Nurse Manager in our organisation is to oversee the delivery of high-quality care and support services to individuals with intellectual disabilities. This involves working closely with multidisciplinary teams to ensure seamless processes from referral receipt up to admission, as well as post-admission monitoring for the first three months.
About the Role:
* Support service audits, quality reviews, and tracking of action plans across multiple locations
* Coordinate and attend referral meetings, assessments, and admissions alongside operations teams
* Promote informed decision-making through assessment of new referrals and compatibility checks for existing residents
* Maintain accurate documentation on the Referral Tracker and ADT Pipeline
* Foster effective communication with referring partners regarding referral status updates
