Role Title:Medical Social Worker Job Reference Number:002353 Closing Date:Monday, 1st June 2026 Purpose of the Role: To provide a direct social work service to patients and relatives who are dealing with the practical and psychosocial impact of illness. Key services include counselling, risk and need assessments, accessing community resources and entitlements, and arranging safe effective discharge from hospital. This role/grade will be assigned on a rotation basis within the Social Work Department. Provide supportive counselling to patients and families during their illness and treatment. Complete psychosocial assessments and care plans in conjunction with the patient according to their will and preference. Work with the patient/family to implement the care plan in an effective and timely manner. Actively engage in multidisciplinary team work and case consultations, at a hospital and community level, in order to implement comprehensive care plans and safe discharge for patients. Respond to safeguarding concerns in line with relevant legislation and policy. To evaluate ones practice in accordance with evidence-based standards and to initiate improvements. To maintain high standards of record keeping in line with the guidelines of the Hospital and the Social Work Department. To carry out home visits as appropriate. To engage in regular supervision in accordance with departmental policies and to commit to a plan for ongoing professional development. To contribute to any research projects involving the Social Work Department. To carry out any other duties appropriate to the office as may be assigned by the Principal Social Worker. Participate and cooperate with legislative and regulatory requirements in relation to Health and Safety as outlined in the Safety, Health and Welfare Act 2005 and the Hospital Safety Statement. Ensure own mandatory learning and training is kept up to date. Required: Candidates for appointment must: Be registered as a Medical Social Worker by the Health and Social Care Professionals Council at CORU. See Or Currently leading to successful completion of a qualification approved by the Social Workers Registration Board at CORU and will be eligible to apply as a newly qualified graduate for CORU registration in 2026. And Provide proof of Statutory Registration on the Social Worker Register maintained by the Health and Social Care Professionals Registration Board at CORU before a contract of employment can be issued. And Candidates must have the requisite knowledge and ability (including a high standard of suitability and professional ability) for the proper discharge of the duties of the office. Annual Registration: On appointment, practitioners must maintain annual registration on the Health and Social Care Professionals Register maintained by the Health and Social Care Professionals Registration Board at CORU. And Practitioners must confirm annual registration with CORU to Human Resources, St Jamess Hospital by way of the annual Patient Safety Assurance Certificate (PSAC). And Knowledge of social work theories and application of theories in practice. Understanding of the role of social worker in a medical setting. Understanding of relevant legislation, guidelines and code of ethics that impact on clinical practice. Desired: Medical social work experience.
