Community Nurses Permanent Positions 37.5 hour working week Community Nurse Team Reporting to the Community Nurse Manager, the successful candidate will be delivering responsive person-centred care within the Horizons Community. This team will respond to the evolving health needs of the People We Support (PWS), through compassionate care, innovative approaches and sustainable practices, while actively promoting the health and wellbeing of the PWS across various settings within Horizons.
