Programmes Facilitator
We are seeking a highly skilled and dedicated individual to join our team as a Programmes Facilitator. In this role, you will play a key part in promoting and supporting independent living and community integration through person-centred programmes.
Key Responsibilities:
* Develop and Deliver Programmes: You will develop and deliver programmes to meet the needs of our service users, identified through Person Centred Planning.
* Key Worker Role: As a key worker, you will be responsible for facilitating the Person Centred Planning process with our service users.
* Support and Facilitation: You will support individuals and groups accessing their community by facilitating activities and programmes.
Requirements:
* Minimum QQI Level 5 in Social Care: You must have a minimum QQI Level 5 in Social Care or equivalent qualification.
* New Directions and Community-Based Programmes: You should have good knowledge of New Directions and community-based programmes.
* Intellectual Disabilities and Mental Health Issues: You must have knowledge and understanding of intellectual disabilities and mental health issues.
