Responsibilities
* Motivate, support, and encourage service users to engage in planned activities of their choice.
* Support the care of the service user, including personal care and providing a general overview of their everyday needs.
* Support and promote independence and community integration in line with New Directions (HSE, 2012).
* Take personal responsibility for educating yourself about each service user, listening to them, and using reflective practice to develop your skills and knowledge.
* Participate in a key worker role under guidance from a senior staff member.
* Work as part of a team, supporting colleagues and taking ownership of your workload. Attend regular team meetings and engage in supervisions.
* Maintain confidentiality in accordance with GDPR at all times.
* Manage and administer medication following the medication policy.
* Liaise with multidisciplinary teams, families, and other relevant parties concerning the service user.
* Carry out health and safety observations and reporting.
* Contribute to service users' Person-Centred Planning meetings and follow up on actions and outcomes.
* Support service users in their daily lives and help them make appropriate choices.
* Report concerns regarding welfare, safety, and health.
* Accurately record information in line with National Standards and New Directions.
* Support the management of behaviours of concern using positive behaviour support plans tailored to each individual.
* Contribute to maintaining high-quality service through audits and follow-up actions.
* Follow financial procedures, report discrepancies.
* Build and maintain positive relationships with families and relevant contacts.
* Develop and sustain professional rapport with colleagues to ensure effective service delivery.
* Create opportunities for service users to participate in the community.
* Support service users' volunteer, employment, and educational pursuits.
* Attend mandatory training and development programmes.
* Participate in supervision and appraisals, reflecting on practice to enhance skills.
* Undertake additional work as necessary within the scope of the social care worker role to meet service requirements.
Qualifications
* Minimum Level 7 qualification in Applied Social Care or relevant field.
* Full driver's license with at least two years' experience.
* Currently undertaking or holding a CORU-recognised Social Care Qualification.
