Role Overview

This role involves offering practical assistance, emotional support, information, and guidance to individuals and their families/carers who are living with or adjusting to cognitive health challenges.

The advisor will help connect clients to appropriate services and supports, enabling them to make informed decisions and maintain control over their lives. A key focus of the role is to support clients and families in navigating the health and social care systems, ensuring timely access to the right care and resources.


Key Responsibilities

* Provide person-centred support, advice, and signposting to individuals and their families or carers.
* Assist in navigating complex health and social care systems.
* Facilitate access to relevant services and community-based supports.
* Empower clients to live as independently and meaningfully as possible.


Qualifications

* A primary degree or equivalent qualification in a relevant health or social care discipline.
* Demonstrated knowledge and understanding of cognitive health and related challenges.
* Proven skills and experience in supporting individuals with complex or evolving care needs.
* Strong insight into the impact of cognitive conditions on individuals and families.
* Experience in the community care sector.
* Evidence of continuous professional development.
* Familiarity with legal and ethical considerations in adult safeguarding, including:
o Enduring Power of Attorney
o Ward of Court processes
o Assisted Decision-Making (Capacity) legislation


Personal Attributes

* A strong client focus with a compassionate and empathetic approach.
* Excellent communication, interpersonal, and coordination skills.
* Commitment to providing high-quality, person-led support.
* Ability to work both independently and collaboratively as part of a team.


Other Requirements

* A full, clean driving licence is essential, as the role involves travel.
* Salary will be in line with sector standards and based on relevant experience.
* This position is subject to NDO/HSE funding.
