Job Description
We are seeking a skilled and experienced professional to fill the position of Service Coordinator. The successful candidate will be responsible for developing and running a service for adults with disabilities who require individualized support.
The primary objective of this role is to deliver high-quality services that cater to the unique needs of our clients. To achieve this, the Service Coordinator will work closely with the multi-disciplinary team to ensure seamless coordination and implementation of our service plans.
* Develop and implement individualized support plans for clients
* Manage and coordinate staff supervision, training, and development
* Cultivate strong relationships with families, local communities, and similar organizations
* Maintain accurate records and documentation in line with organizational policies
To succeed in this role, the ideal candidate will possess excellent leadership and communication skills, with a proven ability to work effectively within a team environment. A third-level qualification in education, psychology, or a related field is essential, as well as at least two years' experience in leading a team or coordinating services in the disability sector.

* Positive behavior support planning: Skilled in designing and implementing individual support and learning plans for adults with autism and intellectual disabilities
* Personal planning for individuals: Experienced in assessing individual learning and support needs, working with families, and designing learning programs
* IT proficiency: Excellent knowledge of IT systems and software
