Duration of Post - 1 Year
The successful candidate will form part of a team that will deliver care in the acute and community settings and will enable the development and delivery of the East Galway and Roscommon ICPOP.
The post holder will work with the Frailty at the Front Door Service and Geriatric Medicine Consultation Service in PUH in conjunction with their integrated care roles.
The successful candidate's specialist interest and expertise should include Falls and Syncope, Dementia and Delirium, Stroke Medicine, Polypharmacy, and Frailty in the Emergency Department management, in addition to proficiency in head up tilt table testing, ambulatory blood pressure monitor and external event monitor interpretation.
The post holder will support an integrated service ensuring sufficient leadership capacity to allow ICPOP to be fully leveraged.
The post holder will work in close liaison with the project manager and governance group in developing services and the MDT across community and acute settings.
The local GRICPOP governance Group with operational and strategic governance responsibility will develop standard operating procedures and retain overarching governance over all staff groups working within the integrated service
The geriatric medicine department has a long term vision that all three facets of integrated care for older people (community, ambulatory specialist care and acute care) are available to older people in the Galway/Roscommon area.
The proposed post facilitates the next step in realizing this vision where we can now focus on development of the community geriatric services in tandem with existing developments around the Older Adult Hub and inpatient specialist geriatric services at PUH.
