About the Role
The Access to Care Coordinator will play a pivotal role in enhancing patient care and experience within our healthcare services. As part of an integrated team, this individual will be responsible for streamlining access to care processes, ensuring seamless communication with patients, staff members, and external agencies.
Responsibilities
* Coordinate admissions and ensure efficient communication with nursing teams
* Manage telephone calls professionally and direct inquiries to relevant personnel as needed
* Maintain accurate records on iSOFT system and update information promptly
* Aid in transferring patients between departments or hospitals while maintaining confidentiality and adhering to hospital policies
