About the Role:
We are seeking a skilled and compassionate Dementia Advisor to join our team. The successful candidate will be responsible for providing support, advice, and guidance to individuals and their families affected by dementia.
The role will involve working on an outreach basis, visiting people in their own homes, and supporting them to access appropriate services and care. This will include signposting to local resources, advocating on behalf of clients, and providing emotional support.
As a Dementia Advisor, you will work closely with other healthcare professionals, social workers, and community groups to ensure that individuals receive a holistic approach to their care.
Key Responsibilities:

Requirements:
To be successful in this role, you will need:
