Rehabilitation Support Professional

The role of the Rehabilitation Support Professional is to provide assistance and support to individuals with acquired brain injury on every aspect of activities of daily living, enabling them to live valued lives of optimal independence and participation in community activities.

* Form a therapeutic relationship and rapport with each individual served and maintain a positive constructive and professional relationship.
* Facilitate and coordinate the daily independence of individuals served within the service/community by working with them and their family in formulating Individual Rehabilitation Plans (IRPs).
* Assist and encourage individuals served to achieve their goals as identified in their IRPs.
* Support individuals served in improving their quality of life by developing, encouraging, and supporting them in the activities of daily living, learning of skills for gaining employment, and leisure activities.
* Record and document daily activities, reports, and reviews via electronic systems.
* Work as a member of a team and attend team meetings and reviews as required.

Required Skills and Qualifications:

* A relevant Health or Social Care qualification to a minimum QQI Level 5 standard.
* A person-centred approach to supporting individuals with acquired brain injury.
* Excellent inter-personal and communication skills.
* Must be able to write and speak English to a high standard.
* Good IT skills.
