Job Overview
We are seeking a skilled and compassionate Care Coordinator to join our team. In this role, you will provide comprehensive support to families with children who require specialized care.
Key Responsibilities:
* Implement a preschool program that fosters creativity and development through art, sensory activities, and music circle.
* Establish strong relationships with a set number of families, providing guidance and support as needed.
* Facilitate access to essential services such as equipment and home support/respite for families.
* Work collaboratively with families to implement therapeutic programs at home and in the community.
* Participate in interdisciplinary meetings and contribute to the development of Individualized Family Service Plans (IFSPs).
* Document all interactions with families in online records.
* Monitor and report any issues or concerns, contributing to the implementation of relevant programs.
* Support children's participation in community and leisure activities.
* Visit preschools to observe and support child participation.
* Cofacilitate training groups for toileting and sleep skills.
* Organize coffee mornings for parents and facilitate workshops for children.
* Promote awareness and understanding of learning disabilities within the community.
* Contribute to staff instruction and training.
* Ensure adherence to health and safety protocols within the unit.
* Report any faulty equipment or hazards.
* Accurately document accidents and maintain a safe and tidy environment.
