The role involves offering practical assistance, support and advice for individuals and their families, enabling them to take control of their own lives.
Key Responsibilities:
* Provide emotional and practical help to individuals with dementia and their families, starting soon after diagnosis.
* Share information in a way that suits each person best, working with relevant stakeholders as needed.
* Offer additional support during challenging times and provide ongoing support to the whole family when required.
* Collaborate with healthcare providers and community services to ensure seamless care coordination.
* Maintain accurate records of service users and track important data.
* Document all work completed and share regular updates to assess service effectiveness.
Essential Criteria:
* Hold a degree (or equivalent qualification) in a healthcare field such as nursing, occupational therapy or social work.
* Have knowledge about dementia and related topics.
* Understand the impact of dementia on individuals' lives.
* Experience with quality standards and risk management is advantageous.
