Job Title: Community Support Worker
This role involves empowering individuals to live independent lives, developing partnerships and facilitating community access.
* Empower service users to live independently in their homes and the community.
* Develop and deliver person-centred plans with service users.
* Establish community connections to foster active citizenship aligned with service user goals.
* Transport service users to access community resources.
* Perform key worker duties, including weekly administration, advocacy and liaison with families.
* Hold a relevant QQI qualification (minimum Level 5 in Healthcare or equivalent).
* Experience working with individuals in the disability sector.
* Be at least 18 years old.
* Possess a valid manual driving licence with access to own vehicle for work purposes.
* Have excellent spoken and written English communication skills.
* Previous experience working with individuals with disabilities or in relevant sectors is advantageous.
