HSE Disability Services in the HSE Mid West support and enable children, young people and adults with disabilities to maximise their full potential, living ordinary lives as independently as possible within their community.
HSE Mid-West Disability Services encompasses services for children, young people and adults who have an Intellectual Disability, Mental Health Intellectual Disability, Physical and Sensory Disability, Autism or Acquired Brain Injury.
It also includes management/monitoring of Service Level and Grant Aid Agreements through which Section ***** and Private Providers are funded.
It encompasses development and oversight of new and emerging service delivery models.

HSE Children's and Adult Disability Case Management Service is responsible for the planning, arranging and subsequent co-ordination and delivery of services using individually tailored care plans, having a person-centered approach and a multi-disciplinary focus.
HSE Disability Services deliver on a wide range of required programmes and initiatives including; Assessment of Need as per the Disability Act, Family Centered Practice, New Directions Model for Day Services, De-Congregation of Residential Campuses and Transition from Nursing Home to Community Care Accommodation.
