You will be based within the Community Rehabilitation Team in Angus which sits within the Angus Health and Social Care Partnership.Our Vision Statement "Working together, developing communities that actively care, promoting wellbeing and creating the best possible health and social care across Angus"The post holder will work within the community rehabilitation team and carry a caseload of community patients across the north of Angus. The caseload is varied including Musculo–skeletal, neurological, frailty and orthopaedic. The team enjoy a close working relationship with other members of the MDT and are actively involved in Enhanced Community Support (ECS) meetings across the locality.The physiotherapy team are actively involved in the Angus Parkinsons disease multi disciplinary annual review clinics as well as the delivery of the better balance programme across Angus.A key requirement of this post is to provide support to the Angus Falls service.
