About the Role:
This role is an integral part of our service, supporting families through their engagement and coordination of personalised care plans.
Main Responsibilities:
* Act as the main point of contact for clients and families throughout their journey from intake to service delivery.
* C coordinate and conduct intake assessments and reviews with clients of all ages. Use digital tools to support effective service delivery and reporting.
* D evelop individualised family goals based on identified needs at referral, intake, client engagements, and review.
Skills & Qualifications:
To be successful in this role you will need excellent communication skills relating to people living with neuro-physical disabilities.
