Ensuring the individual has the opportunity to participate in the local community in line with their care plans and goals. Activities include nature walks, swimming, cinema, gym, and going for coffee. Support Workers require a QQI Level 5 qualification in Health or Social Care or equivalent. Social Care Workers require at least a QQI level 7 qualification in Social Care or related field.The following will be seen as an advantage:One year of experience in a Healthcare or Social Care setting.Experience/knowledge of report writing and client support plans.Candidate Skills / ExperienceThe ability to support the client group in a respectful way, always with the client’s goals and needs at the centre of every task.An understanding of how to prevent and manage challenging behaviour.Strong communication and interpersonal skills.Ability to work on own initiative and as part of a team.Comfortable working with essential information technology and providing reports.Main Duties / ResponsibilitiesSupport service users with an intellectual or physical disability, autism.Practice safe driving habits.Maintain company vehicles in good working order with attention to hygiene and presentation.Alert support team when any item or service is needed for the vehicle.Ensure service users are supported to make sense of their environment, learn new skills and maximise independence.Assist individuals with daily living activities.Support individuals to live a meaningful life, focusing on equal citizenship.Facilitate everyone supported by the services to actively participate and integrate into the community.Act as an advocate for clients or residents.Actively promote the participation of clients or residents in the formulation and execution of their care/placement plan.Accompany service users to appointments and other events as required.Assist service users who have reduced mobility.Undertake duties as may be assigned by your direct manager/team leader to support the care of service users.Manage challenging behaviours, using a positive behaviour support plan.Provide reports on client activities to management.Attend staff meetings and contribute constructively to the ongoing progression and implementation of the care plan.The above Job Description is not intended to be a comprehensive list of all duties involved and consequently, the post holder may be required to perform other duties as appropriate to the post which may be assigned from time to time and to contribute to the development of the post while in office.
