PurposeThe primary role of the Social Care Leader will be one of professional and business leadership in the strategic day to day operations of assigned Dídean homesManage the staff team to provide the highest quality of support to residents in their assigned homesProvide support to staff through daily interaction, supervision, performance appraisal and monthly team meetingsDeliver high quality, safe, effective, person-centred front-line care, and services to all residentsSupport the staff team by dealing with queries, facilitating access to resources, addressing issues with individual residents, follow up on actions or concerns, providing on the ground support and guidanceAssign key workers to residents and ensure the implementation of Family Integration Plans (FIP's).Responsibilities· Ensure that the standard of support and care provided to residents is consistent with the Mission, Vision, and Values of Dídean· Adhere to evidence-based practice, using a person focused care planning approach to care· Maintain a safe environment for residents, staff, and visitors· Take a high level of ownership within your assigned homes· Establish and maintain relationships with residents based on respect and equality· Participate in the development and implementation of Dídean's Quality and Safety Management System (incident, accident, risk and complaints management, audits, quality initiatives etc.)· Promote good relations with the neighbours, community resources and other agencies towards the promotion and development of services· Report any accidents, incidents, and complaints to the Deputy CEO· Participate as a team member of the multi-disciplinary care team in all aspects in the delivery of resident care· Ensure compliance with the corporate quality/risk management programmes to ensure the continual development of a high-quality evidence-based service· Be aware of the confidential nature of the work and in particular, the right to resident's confidentiality.· Adhere to the Safety, Health, and Welfare at work policy of the organisation· Take on staff team activities willingly and with enthusiasm· Display initiative by thinking of and recommending suitable improvements to programmes· Support the service in motivating the staff team to work with a common sense of purpose· Undertake such other duties as may be assigned from time to time by the Deputy CEO· Ensure compliance with the Dídean's Digital and Media Policy and Social Media Policy. Ensure that each family has a Family Integration Plan for the resident which reflects the resident's needs, as per their families assessed needs. Ensure the implementation and the development of staff performance through the service's Performance Appraisal system. Ensure there is a planned and actual staff rota that it is properly maintained. Ensure that correct team hours are submitted for payroll.. Ensure that Social Care Workers and Assistant Support Workers are supported to attend training as required.Undertake professional supervision with staff. Attend staff training, seminars, and conferences etc. at the request of the organisation.
