Key Responsibilities

* Provide support and assistance to service users in their daily lives
* Work collaboratively with colleagues to develop person-centred plans
* Maintain accurate records and reports as required

Requirements
To be successful in this role, you will need:
* A full QQI level 5 major award in Health/Care or equivalent qualification
* Awareness of key worker/support worker role and philosophy surrounding support to people with complex needs
* Ability to work on own initiative and as part of a team
* Fluent English and excellent communication skills
