Key Responsibilities:
">
* Facilitate person-centred planning processes to identify goals and supports required by individual service users.
">
* Develop and deliver person-centred programmes to meet the needs of service users as identified through person-centred planning.
">
* Support individuals and groups in accessing their community to meet identified needs by facilitating activities and programmes.
">
* Liaise with relevant team members to ensure that service users receive the highest standard of service.
">
* Communicate with external agencies and families, maintaining relevant records.
">
* Ensure adherence to health and safety legislation and maintain the highest standards of safety, hygiene, and good housekeeping within the service.
">
* Ensure safe transportation of clients when using transport, adhering to the use of all safety equipment as appropriate.
">
* Approach all interactions with service users in a professional manner, with dignity and respect, reporting any concerns to the Programmes Supervisor or Community Services Manager as appropriate.

Requirements:
">
We are looking for a candidate with a minimum of one year's experience working in a relevant setting, including experience of facilitating and implementing person-centred plans, designing, developing, and providing programmes and activities to meet the needs, choices, and abilities of service users.
">
Experience of lone working, supporting people in the community, and supporting adults with autism and mental health difficulties is also essential.
">
A full driving licence is necessary, and candidates must be able to travel. A QQI Level 5 in Social Care or equivalent is also required.
">
Candidates with a third-level graduate degree in Health & Social Care or equivalent may be preferred, along with programme facilitation skills and client handling experience.
