About the Role:
We are seeking a Relief Programmes Facilitator to work in Longford Autism Day Service.
Your Responsibilities:
1. To act as a key worker and facilitate the Person Centred Planning process for service users.
2. To identify goals and supports needed by individual service users and maintain accurate records.
3. To develop and deliver person centred programmes tailored to meet the needs of service users, as identified through Person Centred Planning, and maintain required records.
4. To support individuals and groups in accessing their community to meet identified needs by facilitating activities and programmes and any other reasonable duties as deemed appropriate by the Programmes Supervisor / Community Services Manager.
Key Skills and Qualifications:
* A minimum 1-year experience of working in a relevant setting with expertise in facilitating and implementing person-centred plans.
* Experience in designing, developing and providing programmes and activities to meet the needs, choices and abilities of service users, including providing support in independent living skills and community integration for individuals with intellectual disability.
* A full driving licence is essential, as this role requires travel.
* A minimum QQI Level 5 in Social Care or equivalent qualification is necessary.
* A third level graduate in Health & Social Care or an equivalent qualification is desirable.
