About The Role
The Senior Support Coordinator plays a vital part in our support team, ensuring the physical, emotional, and practical needs of individuals are met. This role is responsible for designing individualized support programs and coordinating their delivery through a team of Support Workers and themselves.
Key Responsibilities:
* Plan, coordinate, and implement the safe transition of new individuals into our service and community.
* Liaise with families, multidisciplinary teams, community groups, and voluntary bodies to promote the needs of those we support.
* Develop and implement individual and group support programs to enhance capabilities and independence.
Requirements:
1. Two years' experience in health and social care, with a minimum QQI Level 5 in Health Care or equivalent.
2. Strong communication, report writing, assessment, evaluation, computer, risk assessment, leadership, problem-solving, organizational, and time management skills.
3. A team player with an understanding of diversity and ability to work flexibly.
