Service Delivery Facilitator

* The Service Delivery Facilitator will work closely with the Community Service Manager and Programmes Supervisor to facilitate service users in their choices, developing programmes within a person-centred model of service delivery.
Programmes may include independent living, personal development, and occupational and leisure activities within a quality-of-life model.
We are seeking someone with at least 1-year experience in a relevant setting, facilitating and implementing person-centred plans, designing, developing, and providing programmes and activities to meet the needs, choices, and abilities of service users.
Key responsibilities include:
* Acting as a key worker to service users to facilitate the Person Centred Planning process.
* Identifying goals and supports required by individual service users and maintaining records as required.
* Developing and delivering person-centred programmes to meet the needs of service users.
* Supporting individuals and groups in accessing their community to meet identified needs.
* Liaising with relevant team members to ensure service users receive the highest standard of service.
Additional requirements include holding a full driving licence, having a minimum QQI Level 5 in Social Care or equivalent, and being flexible with regard to working hours.
