Support Worker
We are seeking a dedicated and compassionate Support Worker to join our team. As a Key Worker, you will be responsible for providing a person-centered service to clients with disabilities and their families.
Responsibilities:
* Develop and implement individualized support plans for clients;
* Liaise with the Manager/Assistant Manager regarding client referrals and transitions;
* Collaborate with clients, document all client engagement, intervention, support plans, progress notes, reports referrals to agreed professional standards;
Required Skills & Qualifications:
A minimum of one year's experience working in the field of disability education or training is essential. A third-level degree in Social Care Education or Health is also required.
