Care Worker Job Description
We are seeking a dedicated and compassionate Care Worker to join our team in delivering person-centred care to individuals with dementia in their own homes.
* Work as part of a multidisciplinary team to provide high-quality care and support services to clients and their families.
* Develop and maintain strong relationships with clients, carers, and healthcare professionals to ensure seamless care delivery.
* Maintain accurate and up-to-date records of client interactions, including care plans, progress notes, and medication administration.

Required Skills and Qualifications
* Degree or equivalent qualification in a relevant field such as social work, nursing, or health sciences.
* Minimum 6 months' experience in a care setting, preferably working with individuals with dementia.
* Excellent communication and interpersonal skills, with ability to build rapport with clients, carers, and healthcare professionals.
* Ability to work independently and as part of a team, prioritising tasks and managing multiple responsibilities.
* Valid driver's license and access to a reliable vehicle.
Benefits
This is a permanent contract of employment working 18 hours per week. The successful candidate will be responsible for providing person-centred care to people with dementia in the client's own home. You will need good communication and organisational skills and be able to work on your own initiative.
