Post Title:

Candidate Advanced Nurse Practitioner/ Registered Advanced Nurse Practitioner in venous thromboembolism (VTE).

Professional Reporting Relationship: Professionally reporting to Director of Nursing (Hospital site) and line management to Directorate Nurse manger Medical 1.

Clinical Reporting Relationship: Clinically reporting to Consultant Haematologist

Salary:

Appointment will be made on the Candidate ANP salary scale (€70,025 - €79,081) or the Registered ANP salary scale (€70,701 - €87,250) at a point in line with Government pay policy.

Hours of work:

Full Time, 37.5 Hours per week

Purpose of the Post

• To contribute to the continuous improvement of the quality of care experienced by patients with venous thromboembolism, as well as other disorders of coagulation (i.e. arterial thrombosis, bleeding and anticoagulant-related issues).

• The specific contribution of the cANP/RANP is to improve outcomes for patients who fall within the cANP/RANP scope of practice.

• The cANP/RANP will compliment and contribute to the overall service provided by the Department of Haematology.

• The cANP/RANP will contribute towards the department’s aim of making Beaumont Hospital a centre of excellence for the management of patients with venous thromboembolism (VTE).

• The cANP/RANP will develop services for women’s health and coagulation haematology in Beaumont Hospital, with a focus on the interplay between VTE, anticoagulation, hormonal contraception, menopausal hormone therapy and heavy menstrual bleeding.

• To assist in the reduction of Beaumont Hospital’s Haematology outpatient waiting lists.

• To ensure that patients with acute VTE events have early follow-up post-discharge. This is in accordance with the international best practice, and serves the dual purposes of patient education and identifying anticoagulation-related issues early.

• To develop an ambulatory DVT pathway, with the purpose of improving patient safety, reducing emergency department wait times, reducing referrals to the medical on call teams and reducing medical outpatient waiting lists.

• To develop the interface between primary and secondary care services (General Practice, External Hospitals, PHNs, and Practice Nurses) through clinical work, education, research and leadership.

• To develop and oversee a safe pathway for the early discharge and community management of patients with low-risk pulmonary emboli, thus reducing the number inpatient hospital bed days.

• To develop education programmes for clinical staff, patients, and their families in Beaumont Hospital, with a focus on the prevention and management of Venous Thromboembolism and its complications.

• In line with Beaumont Hospital’s position as a designated cancer centre, and the fact that thrombosis is the second leading cause of death in cancer patients, the cANP/RANP venous thromboembolism would be expected to develop expertise in cancer associated thrombosis (CAT).

• To advance the theory and practice of advanced nursing practice through demonstrating expert practice, autonomy in practice clinical leadership, research, and audit.

• To work with the healthcare team contributing to meeting health service quality/patient safety and resource needs at local and national level and within a patient-centred philosophy.


Responsibilities

Principal Duties and Responsibilities

Autonomy in clinical Practice

• Identify through collaboration with key stakeholders an agreed patient caseload that the Registered Advanced Nurse Practitioner will manage autonomously within his/her scope of practice once she/he meets the standards and criteria required by the Nursing & Midwifery Board of Ireland (NMBI) to register as an RCANP/RANP.

• Be willing to participate in Clinical Supervision to utilise reflective practice to improve practice and be required to meet the registration requirements and agree and commit to a plan of work.

• Review the scope of practice for the cANP/RANP (Coagulation Haematology) with the key stakeholders within the service such as, Consultant, Director of Nursing, Clinical Nurse Specialists (CNS) and other members of the multi-disciplinary (MDT) team as appropriate.

• Utilise practical and theoretical knowledge and critical thinking skills to a level that will be seen as exemplary amongst peers and will be recognised and acknowledged as an authority in the area of Coagulation Haematology care as agreed for the role.

• Acknowledge and utilise the clinical governance structures within and outside the organisation that support the role of advanced nursing practice in Coagulation Haematology services.

• Review collaboratively agreed clinical decision-making frameworks, protocols, policies, guidelines and referral pathways to support the role responsibilities of the cANP/RANP (venous thromboembolism).

• Develop and obtain the core and specialist clinical skills and competencies to become an autonomous advanced nurse practitioner (venous thromboembolism) whilst recognising limitation to the scope of practice.• Develop close links with all members of the MDT team to ensure that a safe quality driven evidence-based service is being delivered to all patients including the family/carers within their caseload.

• Develop the ability to use professional judgement, clinical reasoning and decision making processes to refer patients to other health care professionals as required and as per agreed referral pathways.• Utilise and obtain formally, if appropriate, practical clinical skills and knowledge to meet patient need, for example Registered Nurse Prescriber and Ionising Radiation Prescriber.

Expertise in clinical practice

The cANP/RANP will:

• Articulate and rationalise the concept of advanced nursing practice within the framework of relevant laws, rules, legislation and guidelines that govern nursing practice including, but not exclusively, The Scope of Nursing and Midwifery Practice Framework (Nursing & Midwifery Board of Ireland (NMBI) 2015), The Code of Professional Conduct for Registered Nurses and Registered Midwives Nursing & Midwifery Board of Ireland (NMBI) 2014), Guidelines for Midwives (An Bord Altranais 2000), The Nurses and Midwives Act (Government of Ireland 2011).

• Demonstrate advanced practical and theoretical knowledge and critical thinking and decision-making skills to develop the skills and competencies to progress towards managing an agreed patient caseload effectively once registered as a cANP/RANP (venous thromboembolism).

• Develop a means of improving practice through exploring practice issues with team members, development of practice protocols and research and audit initiatives.

• Develop skills in order to function in a variety of roles including clinical and consultative, clinical practice development, teacher, coach, mentor, manager and researcher.

• Assess, plan, implement, manage and evaluate patient care holistically, encouraging the patient and family/carers to be involved in their care pathway.

• Identify health promotion priorities and opportunities in the area of Coagulation Haematology and the wider health care agenda as appropriate.

• Initiate timely and appropriate consultation or referral with members of the multidisciplinary team and other professionals as indicated.

• Lead on and participate in case review and debriefing activities as required.

• Evaluate care and lead practice development through multidisciplinary research and audit of practice.

• Act as consultant and educator to the patient, family /carers and the multidisciplinary team in all matters relating to adult venous thromboembolism care.

• Examine methods of improving practice through exploration of clinical practice issues with the multidisciplinary team and collaboratively develop clinical practice protocols and research/audit initiatives.

Professional and Clinical Leadership

The cANP/RANP will:

• Articulate and communicate the vision of advanced level nursing practice for venous thromboembolism.

• Contribute to professional development of nursing, the development of health policy at local, regional and national level in Coagulation Haematology services and the wider health care field.

• Provide professional leadership, strategic direction, expert advice, and support to nursing and the wider MDT at local and national level in relation to venous thromboembolism services development.

• Actively engage in leading and managing change in the delivery of venous thromboembolism services according to patient need and service demand through professional leadership, providing expert nursing advice and support to colleagues and the wider MDT and other key stakeholders.

• Act as a role model of advanced nursing practice for all nursing colleagues, the MDT and other key stakeholders; this includes education & clinical skill development in venous thromboembolism care.

• Assess and evaluate the patient and family/carers experience with the service through evidenced based methods.

• Ensure that effective patient quality safety procedures are in place to comply with local and national requirements.

• Adhere to the mandatory training requirements of the role.

Research

The cANP/RANP will:

• Identify research priorities specific to/related to the area of clinical practice and the scope of practice/caseload

• Lead, conduct, disseminate and publish nursing research to shape and advance nursing practice, education, and policy in relation to venous thromboembolism care and where appropriate the wider health agenda.

• Collaborate with MDT colleagues were appropriate to enhance care provision.

• Identify, critically analyse disseminate nursing and other evidence into clinical practice in the specialist area.

• Engage in a forum which evaluates quality and standards in clinical practice and leads on the promotion of effective clinical standards.

• Develop skills to lead and engage in the review of accidents, incidents and near misses in specialist area. Undertake analysis as appropriate and furnish reports on same.• Initiate, participate in, and evaluate audit of the patient caseload and other relevant areas appropriate to the specialist area. Utilise outcomes appropriately to enhance service delivery.

• Initiate and foster links with third level education institutions as appropriate.

• Contribute to service planning and budgetary processes through use of research, audit, and evaluation processes.

Professional Education

• The cANP/RANP must successfully meet the criteria and standards as required by The Nursing and Midwifery Board of Ireland (NMBI) for registration as an cANP/RANP.

• Have the ability to link the theoretical concepts of cANP/RANP (NMBI, 2010) to practice, make complex clinical decisions and choose from a wide range of alternatives for patient care.

• Demonstrate ability to teach members of the MDT and have proficiency in presentation skills.

• Have knowledge and experience of participating, undertaking and utilizing research and audit both within nursing and multidisciplinary basis

Clinical Governance

The cANP/RANP will:

• Participate in the organisations clinical governance programme as defined by the Director of Nursing and Midwifery

• Contribute to the development of key performance indicators (KPI) and outcome measures relevant to the Haematology services.

• Assist in the monitoring of KPI’s and respond to performance improvement initiatives as required.

• Take appropriate action on any matter identified as being detrimental to staff and/or service user’s care or well- being or which may be inhibiting the safe and efficient provision of care

• Ensure adherence to the organisations policies and procedures on patient safety, infection control and storage and use of medicinal products.

• Ensure compliance with the quality and risk safety framework programme and any culture programmes.

• Establish a feedback mechanism with the organisations risk advisors and report any risk issues to the Director of Nursing and Midwifery Services as appropriate.

• Participate in accreditation programmes external and internal to the HSE.

• Ensure adherence to hospital policies in relation to the care and safety of equipment utilised in the fulfilment of clinical duty.


Qualifications

Selection Criteria

Selection criteria outline the qualifications, skills, knowledge and/or experience that the successful candidate would need to demonstrate for successful discharge of the responsibilities of the post.

Mandatory (Candidate ANP)

1. Statutory Registration, Professional Qualifications, Experience, etc(a) Eligible applicants will be those who on the closing date for the competition:(i) Be a registered nurse with the Nursing and Midwifery Board of Ireland [NMBI](Bord Altranais agus Cnáimhseachais na hÉireann) or entitled to be soregistered.(ii) Be registered in the division(s) of the Nursing and Midwifery Board of Ireland(Bord Altranais agus Cnáimhseachais na hÉireann) Register for which theapplication is being made or entitled to be so registered.In recognition of services that span several patient/client groups and/ordivision(s) of the register, provide evidence of validated competences relevantto the context of practice.(iii) Have a broad base of clinical experience relevant to the advanced field ofpractice(iv) Be eligible to undertake a Master’s Degree (or higher) in Nursing or a Master’sDegree, which is relevant, or applicable, to the advanced field of practice. TheMaster’s programme must be at Level 9 on the National Framework ofQualifications (Quality & Qualifications Ireland), or equivalent. Educationalpreparation must include at least three modular components pertaining to therelevant area of advanced practice, in addition to clinical practicum.OR(v) Be currently undertaking a Master’s Degree in Nursing (Advanced PracticePathway) or be eligible to register to undertake additional Level 9 NationalFramework of Qualifications (Quality and Qualifications Ireland) specific modulesof a Master’s Degree in Nursing (Advanced Practice Pathway) within an agreedtimeframe. Educational preparation must include at least three modularcomponents pertaining to the relevant area of advanced practice, in addition toclinical practicum.

OR(vi) Possess a Master’s Degree (or higher) in Nursing or a Master’s Degree which isrelevant, or applicable, to the advanced field of practice. The Master’sprogramme must be at Level 9 on the National Framework of Qualifications(Quality & Qualifications Ireland), or equivalent. Educational preparation mustinclude at least three modular components pertaining to the relevant area ofadvanced practice, in addition to clinical practicum.(b) Candidates must possess the requisite knowledge and ability including a high standard ofsuitability and clinical, professional and administrative capacity to properly discharge thefunctions of the role.

2. Annual registration(i) Practitioners must maintain live annual registration on the appropriate/relevant Division ofthe register of Nurses and Midwives maintained by the Nursing and Midwifery Board ofIreland [NMBI] (Bord Altranais agus Cnáimhseachais na hÉireann) for the role.(ii) Confirm annual registration with NMBI to the HSE by way of the annual Patient SafetyAssurance Certificate (PSAC).

3. HealthCandidates for and any person holding the office must be fully competent and capable ofundertaking the duties attached to the office and be in a state of health such as would indicate areasonable prospect of ability to render regular and efficient service.

4. CharacterCandidates for and any person holding the office must be of good character.
