Job Title: Community Support Coordinator
We are a specialist voluntary organisation dedicated to empowering individuals with disabilities.
Mission Statement:
* Promote and support independent living and community integration for people with disabilities.
Key Responsibilities:
* Develop, implement and deliver person-centred programmes tailored to meet the needs of service users.
* Provide key worker support to service users through the Person Centred Planning process.
* Facilitate access to community services and resources to meet identified needs.
Requirements:
Minimum Qualifications: QQI Level 5 in Social Care or equivalent. Relevant work experience is essential.
Skills and Attributes: Strong interpersonal skills, ability to work independently and as part of a team, excellent communication and organisational skills.
