Job Purpose
As an Administrator, you will be responsible for completing all assigned TCU administrative tasks for the care centre.

About The Role
Roles & Responsibilities
* Keep database updated with admissions, discharges and personal information.
* Filing and maintaining records including supporting with HIQA compliance, policies and procedures, statutory files and records. Coordinate with Head Office where required.
* Maintain up to date Resident records – Contracts/ funding approvals/ Admissions/ Absences/ Discharges/ RIP's/ HSE/ Pensions/ GP/ Pharmacy/ Medical cards
* Establish excellent professional relationships with the Residents based on respect, rapport, warmth, dignity and professional boundaries
* Work with the multi-disciplinary team in the relevant areas
* Meet and great all TCU patients, issue Patient booklet
* Prepare and issue Contracts of Care
* Preparing reports related to the progress of discharge plan for each patients
* Work within current legislation, relevant policies, and procedures, guidelines, and protocols as laid down by the employer.
* Working closely with "Riverstick TCU Assistant Director of Nursing " to provide support to patients and families, including, discharge planning and follow up or referral on, as required, to statutory and/or voluntary services
* Attend multidisciplinary team meetings and preparing reports as required.(delete as repeated further on)
* Coordinate and minute Resident Forum Meetings
* Prepare patient listing, coordinate the MDT and record MDT minutes
* Compile documentation and forward to GP's PHN and community network team
* Working to support the coordination of transfer to the TCU to ensure patent flow in the centre as required
* Maintain patient data board to maintain patent flow.
* Keep Line Manager fully informed and up-to-date on all significant matters.
* Ensure National Safeguarding polices is adhered to and referrals made to the relevant HSE safeguarding Office.
* Ensure data collection for weekly / monthly report submission to HSE / CUH i.e. DTOC list, bed allocation KPI related data
* Preparation patient admission checklist and robust handover of discharge plan is received and coordinate with "Riverstick TCU ADONs".
* Review demographic details on epic and ensure all patient data on system to maintain Resident register.
* Ensure patients' monies are stored safely in accordance with Safekeeping procedures.
* Arrange and track transport for all admissions and discharges ensuring accurate documentation and coordination.
From time to time you may be required to support with tasks as outlined below:
General Administration
* Secretarial support, answering phones, greeting visitors, answering queries, and providing information to team members, residents and families as required.
* Provide administrative support to the DON/ADON extending to entire department as directed.
* Maintain up-to-date records.
* Dealing with all correspondence, emails, calls, reporting etc.
* Other regular/Ad hoc duties as required.
