Position Summary
The Licensed Practical Nurse (LPN) works under the guidance of the RN Charge Nurse to provide direct nursing care in a hospice setting, following an individualized plan of care developed collaboratively with the patient, family, physician, and hospice team. This position requires providing holistic care addressing physical, emotional, and spiritual needs, as well as supporting families during end-of-life care.
This is a part-time position, requiring two 12-hour shifts per week, with weekend and holiday rotations.
Key Responsibilities
1. Implement skilled and technical patient/family care activities according to the individualized plan of care, as directed by the RN.
2. Perform physical examinations and patient observations on each shift, reporting changes to the RN.
3. Participate in patient and family teaching, ensuring understanding of care plans and promoting independence.
4. Collaborate with the hospice team to address physical, emotional, and spiritual needs of patients and their families.
5. Provide personal care services, including bathing, grooming, dressing, feeding, and assisting with mobility and elimination.
6. Check vital signs, perform skin and oral hygiene, and maintain accurate records.
7. Maintain effective communication with patients, families, physicians, and team members.
8. Work flexible shifts, including weekends, and participate in on-call responsibilities as assigned.
Qualifications
* Current LPN license in Pennsylvania.
* At least two years of patient care experience preferred.
* Excellent clinical skills.
* Current CPR certification.
* Strong organizational skills and attention to detail.
* Effective written and verbal communication skills.
