This permanent, full-time position offers the opportunity to support patients and families during a critical stage of recovery following acute illness, surgery, or major health events.
1. • Provide comprehensive psychosocial assessment and intervention to patients and their families within the Transitional Care setting.
2. • Support patients in navigating complex social, emotional, challenges related to returning home from hospital.
