Job Title
A Community Support Worker role is available in our team, providing 12 months of fixed term employment. You will work 30 hours per week.
About the Role
This position involves facilitating internal and external programs within our service, including vehicle escort support for users traveling to and from their homes.
You will also provide personal care where required, conduct key worker meetings and updates, and assist with person-centered planning.

Our Requirements
We are seeking a Community Support Worker who has:
* Minimum QQI Level 5 in Social Care or equivalent qualification
* Experience supporting adults with an intellectual disability and those with mental health issues
* Community support skills
* Experience working in a relevant setting with experience of facilitating and implementing person-centered plans
* Good knowledge of community-based programs
Your Responsibilities
To be a key worker to users in order to facilitate the Person-Centred Planning process.
Identify goals and supports required by individual users and maintain records as necessary.
Develop and deliver person-centered programs to meet the needs of users through Person Centred Planning.
Support individuals and groups in accessing their community to meet identified needs by facilitating activities and programs.
Work independently and liaise with team members to ensure users receive the highest standard of service.
Communicate with external agencies and families, maintaining records.
Observe all rules and regulations ensuring compliance with policies and procedures.
Promote independence and community integration in your approach with the user group.
Comply with health and safety legislation and maintain high standards of safety and hygiene.
