Role Purpose:This Urgent care post sits within a dynamic and diverse multi-professional team driving innovation in a primary care model based on the patient centered medical approach.You will join a multidisciplinary team of health professionals including a clinical Consultant lead with background in General Surgery, nurses in the delivery of effective and high quality primary care, encompassing medical screening to all patientsThe role has scope for national delivery of care, however your primary base will be at the Urgent Care Site located in Carlow town.There will be opportunity for the successful post-holder to develop and maintain sub-speciality interests.Key ResponsibilitiesEngage with other primary care colleagues within the Urgent care network to provide enhanced ambulatory community care to address patient needs.Support an integrated care approach across the organisation and related service including but not limited to urgent care.Promote and develop disease prevention measures.Champion clinical excellence and patient-centric healthcare engaging voice of customer and capturing patient reported outcomesTo participate in the delivery of effective clinical governance and to ensure that processes are in place which address clinical effectiveness and outcomes, clinical risk, clinical incidents, patient experience and complaints.Work with the relevant Quality, Safety & Risk (QSR) team members to ensure that the design and delivery of enhanced primary care services adhere to H&W QSR policies, procedures, protocols and guidelines (PPPGs) and external standards / statutory requirements (e.g. HIQA).To identify education requirements and opportunities and provide education to staff.Review and interpret results and ensure critical results are escalated appropriately.Complete medical reports for patients based on agreed protocols.Communicate to patients, provide relevant lifestyle advice and ensure patient understanding of their results.Participate in clinical audits, quality improvement and patient safety initiatives.Work as part of a team to ensure patients receive excellent clinical and customer-focused experience.Participate in clinical meetings and stay up-to-date with latest evidence based clinical practice and guidelines.Provide advice to other clinical colleagues when or as required as set out in SOPs.Maintain ongoing CME.Undertake any other clinical duties as may arise from time to time within the clinicParticipate in and support any accreditation programme through compliance with SOPs and clinical best practice, as laid out in the relevant standards.The above is not intended to be a comprehensive list of all duties involved and consequently, the post holder may be required to perform other duties as appropriate to the post, which may be assigned to them from time to time and to contribute to the development of the post while in office.Knowledge & ExperienceTechnical:Experience of developing, applying and reviewing an evidence-based approach to decision-makingExcellent communication skills, both verbal and writtenDemonstrate evidence of effective planning and organising skills and has the ability to manage multiple complex tasks.Ability to analyze and interpret data to measure success of health and wellness initiatives.Understands the principles of efficient patient flow.Demonstrate a commitment to fostering partnerships, multi-professional and multi- disciplinary relationshipsDemonstration of research and clinical excellence through peer reviewed research and publications desirableInterpersonal:Operate with professional and personal integrity coupled with an overarching humility and proven ability to build strong working relationships.Demonstrate an interest in, and be an advocate for all aspects of patient careDemonstrates a match to values of health and wellbeing including commitment to care for patients by leading with heart/ courage to create a new future and together we do our best workEssentialHold specialist registration with the Irish Medial Council, in the division of General PracticeMinimum 5 years relevant clinical experience (inclusive of GP Training).Experience working with multi-professional, multi-disciplinary teamsExperience facilitating patient pathways from community to tertiary careDesirablePostgraduate training in area of interest such as Women's Health, Paediatrics etc.Advocate of value based care, experience of developing patient reported outcomes and delivering outcome based clinical practice.Job Type: Full-timePay: Up to €100.00 per hourBenefits:Company pensionPrivate dental insuranceRelocation assistanceExperience:Emergency Doctor: 2 years (preferred)Language:Fluent English (required)Work authorisation:Ireland (required)Work Location: In person
