Haemovigilance Officer - CNMII Location: UPMC Whitfield Hospital Job type: Permanent Overview of role: To embrace the five core concepts to ensure the provision of high quality, holistic and integrated transfusion service for all patients. To ensure adherence to the requirements of the EU Blood Directive 2002/98/EC and the ISO Standard 15189 throughout the entire transfusion process, assessing current treatments, improving the management and clinical outcomes of care, monitoring cost effectiveness in care provision, and supporting practice development and education to facilitate the development of competencies within the speciality day unit services. To work in collaboration with the Director of Nursing, Quality Manager and other members of the Multi - Disciplinary team. Primary Duties and Responsibilities: Clinical: The post holder will provide a Transfusion Surveillance Service for UPMC Whitfield Hospital as part of the Blood Transfusion Department Act as a positive clinical role model and provide support and clinical advice to staff. Demonstrate expert practice where appropriate. Lead by example through visible leadership to motivate and empower others To ensure that relationships and own behaviour is managed in line with UPMC Whitfield Hospital PPPG's Will have knowledge of and adherence to the following: Directive 2002/98/EC of the European Parliament and of the Council of 27th January 2003 Minimum Requirements for Blood Bank Compliance with Article 14 (Traceability) and Article 15 (Notification of Serious Adverse Reactions and Events) of EU Directive 2002/98/EC (AML-BB) ISO Standard 15189 - Medical Laboratories - Particular requirements for quality and competence University Hospital Waterford - Quality Manual WRH-PATH-LP-104 Haemovigilance Handbook - Requirements for Reporting Serious Adverse Reactions and Events to the National Haemovigilance Office. Version 1.13 Haemovigilance Standard Operating Procedures Lead out on the accreditation process and to provide all relevant data for Irish National Accreditation Board (INAB) as required for all inspections and surveillance inspection visits. Actively take the lead for CHKS / JCI / HIQA /JAG etc. accreditation processes for haemovigillance compliance and work with both quality and nursing to ensure adherence to current and future guidelines Promote and facilitate adherence to best practice in all aspects of transfusion process in particular clinical transfusion practice, including positive patient identification at time of pre-transfusion sampling, and administration of blood components and blood products Devise, implement and update PPPG's standard operating with regard to the transfusion process in conjunction with the Consultant Haematologist, Director of Nursing, Laboratory Manager and Laboratory Quality Officer and the Hospital Transfusion Committee (HTC) for use in UPMC Whitfield Hospital /UHW. Establish complete transfusion traceability and maintain traceability records as per Article 14 of the EU Directive 2002/98/EC. 1 The Haemovigilance Officers devises, implements, maintains, and updates an effective traceability system in conjunction with the Blood Transfusion Laboratory Staff. 2 Investigation and follow-up of all non-conformances/deviations detected in the traceability system are recorded by the Blood Transfusion Medical Scientists on the Blood/Product and Traceability Issues for Haemovigilance. Appropriate corrective and preventative actions are implemented as necessary by the Haemovigilance Officers following investigation. Maintain the Blood Tracks system in conjunction with the Lab Manager and ensure proficiency with staff, providing training on a regular basis in all clinical areas Liaise with Blood Transfusion Laboratory staff in investigating all reports of transfusion reactions/events and report same to the Consultant Haematologist and then, if necessary, to the National Haemovigilance Office (NHO) as per Article 15 of EU Directive 2002/98/EC. Record all non-conformances reported to or discovered by clinical staff and Medical Scientists with regard to traceability and Haemovigilance activities, investigate these non-conformances and implement corrective/preventative actions, and the close out all non-conformances where possible. Keep live data on all non - conformances across the clinical areas Design, gather and deliver a high - level reporting structure for the Haemovigillance Service, to be discussed at IQS Advise staff in the clinical area of the requirement to report clinical incidents in relation to the transfusion process which are highlighted to/or identified by Haemovigilance to the Quality & Safety Manager. Reports accidents, incidents, and complaints pertaining to ward / clinical activity in relation to transfusion practices to appropriate personnel. Follow up on all Massive Transfusion events and transfusion events where un-cross matched red cells are issued in an emergency or the Emergency O RhD Neg units are used for a patient. A summary of these events is included in the quarterly HTC report for review. Assist in the collection of clinical and laboratory data on transfusions that can be used for formulation and updating of recommended 'Maximum Surgical Blood Order Schedule' (MSBOS) and cross match/transfusion ratios. Ensure that all procedures conform to Hospital policy and NMBI Regulations The Haemovigilance Officers are members of the Hospital Transfusion Committee (HTC) and attend meetings as organised Comply with Document control procedures and Numbering of Documents. The overall document control system for the Blood Transfusion/Haemovigilance department is overseen by the Director of Quality & Patient Safety. Patient Advocacy: Ensure that regard is taken to the dignity, choices, self-esteem and wellbeing of patients and their relatives in the specialist area of Blood Transfusion and Haemovigilance Ensure that patients/ relatives receive information including information leaflet relating to the transfusion of blood products/components to allow them to give informed consent. In collaboration with patients, take available opportunities to communicate, represent views of and negotiate for an individual or group of patients at a multidisciplinary level, service level and/or with other personnel or agencies as appropriate Develop systems to give service users and carers an opportunity to have views represented in relation to service provision, planning, evaluation, and development. Education and Training: Facilitate and promote best transfusion practice by providing formal and informal training instruction to all staff involved in any aspect of the blood transfusion process. All Staff involved in any aspect of the blood transfusion process i.e. prescribing, collecting, administering blood components/products must receive the relevant training and attend Mandatory Haemovigilance Training every 2 years which is run throughout the year in the hospital for different disciplines. Plan, develop, implement and evaluate the continuing education programmes for clinical staff relating to blood transfusion best practice Liaise with the Clinical Facilitator to ensure that Undergraduate Nursing Degree Students receive education in the area of Haemovigilance and Blood Transfusion. Maintain records of attendance competency assessments and evaluation forms for all staff who attend all education programmes for evaluation purposes. Provide additional individual training/ retraining as required. A mandatory database of all relevant staff who attend Haemovigilance education either formal or informal is maintained and updated by the Clinical coordinator. Take personal responsibility for own professional development, including attendance at National Haemovigilance Annual Conference and maintain records of same Audit and Research: Demonstrate a commitment to evidence-based practice. Actively engage in current developments within Haemovigilance. Use scientific, relevant research findings to influence own practice and practice of others within Haemovigilance Take every reasonable action to ensure practice in the field of Blood Component/ Product Administration and Haemovigilance is evidence based and up to date Incorporate & disseminate relevant, up to date research findings in the area of blood transfusion. Develop and adhere to a yearly Audit Schedule in conjunction with the Laboratory Quality Officer. Take an active role in the audit of clinical practice and standards of care relating to transfusion and Haemovigilance Identify, plan, and participate in audit programmes and other evaluations of the service, in line with local, regional and national developments, to include. 1 vertical audit 2 audit of current practices 3 audit of quality of delivery of the service Review the audit findings, and identify, document, and investigate non-conformances discovered. Identify and implement corrective/preventative actions where possible. The completed audit data is reviewed by the quality officer in blood transfusion. The findings of the audit are summarized for discussion at the Monthly Management Laboratory meetings and for inclusion in the quarterly report to the HTC. Contribute to an annual evaluation of the service. 1 Annual Blood Bank Report to Irish Medicines Board (IMB) in conjunction with Consultant Haematologist, the Laboratory Manager, and the Laboratory Quality Officer. 2 Report to Annual Quality Review 3 Annual Report to NHO - Annual Notification of Serious Adverse Reactions and Events in conjunction with Laboratory Ensure that accurate documentation and record keeping is maintained Consultancy/ Advisory Service: Be available as a clinical resource and consultant, both inter and intra disciplinary, in Haemovigilance, to promote improved patient management and quality of care Follow up and report on all Blood Product Look Back requests Report Suspected Adverse Reaction & Events from clinical areas Report Non-Conformances from clinical area and laboratory Establish and standardise clear communication channels for such consultations, inter and intra disciplinary Professional Accountability and Development: Ensure staff and self, remain professionally updated and ensure care in all areas is delivered in line with the latest research to promote evidence-based practice. Actively participate in research and audits where appropriate for the benefit of patients. Maintain own professional portfolio and participate in the Hospital's clinical supervision programme. Ensure that statutory rules, guidelines and legislation are understood and complied with. Act as a positive role model with regards to the Hospital ethos and UPMC values. Develop and maintain a high level of knowledge of the following and always practice within the guidelines and policies written at Hospital level: Health and Safety: To uphold UPMC Whitfield Hospital's policy requirements in relation to Fire, Infection Control, Radiation Safety and Health and Safety at work regulations and to attend updates. Identify potential risks for all staff, patients and visitors and adhere to relevant incident reporting procedures. To observe a general duty of care for the health, safety and well-being of self, work colleagues, visitors and patients within the hospital, in addition to any specific risk management associated with this post. Qualifications & Experience: Be Registered General Nurse with The Nursing and Midwifery Bord of Ireland (NMBI). At least 3 year's post registration experience in an acute hospital setting - essential.
