Overview
The Assertive Community Treatment (ACT) Case Manager is part of a community-based mental health team providing services for individuals experiencing serious mental illness. The role aims to promote independence and community integration through a team-based service model, addressing mental health, addictions, substance abuse, and self-esteem issues.
Position Details
This community-based role requires at least 80% of services to be rendered in the beneficiary's home or other community settings.
Compensation
Starting at $44,000 for unlicensed candidates; $44,000 - $53,950 for limited licensed candidates; $44,094 - $58,100 for fully licensed candidates, based on skills and experience.

Key Responsibilities
* Provide community-based case management and integrated behavioral health services
* Conduct assessments and develop Person-Centered Plans
* Facilitate group sessions and provide transportation assistance
* Complete documentation accurately and timely
* Coordinate medication and collaborate with healthcare providers
* Utilize motivational interviewing and trauma-informed approaches
* Advocate for individuals and connect them with community resources
* Ensure compliance with policies, HIPAA, and regulatory standards
Qualifications
Required
* Qualified Mental Health Professional (QMHP) with a Bachelor's Degree in a Human Services field or equivalent experience
* Valid Michigan Driver’s License and auto insurance
* Proficiency with Microsoft Office and electronic health records
* Strong interpersonal, communication, and teamwork skills
Preferred
* Master’s Degree in a related field
* Michigan licensure (LBSW, LMSW, LPC, etc.)
