We are seeking a highly skilled and compassionate individual to fill the role of Community Living Specialist.

About the Role:

The purpose of this position is to promote and support independent living and community integration, personal development, and work opportunities for service users. This includes assisting in the development, implementation, and delivery of quality customer-focused and cost-effective community services.

As a Community Living Specialist, you will be responsible for building trusting relationships with service users, facilitating them to identify their goals, strengths, and needs. You will also support service users in establishing and enhancing their natural support circle and identifying practical solutions to any barriers to accessing community-based supports.

Your Responsibilities:

* Build trusting relationships and communication with service users

* Facilitate service users to identify their goals, strengths, and needs

* Support service users to establish and enhance their natural support circle

* Identify practical solutions to any barriers to accessing community-based supports

* Provide accessible information on available community-based supports

* Audit and maintain activities and supports at an appropriate pace

* Advocate on behalf of and support self-advocacy for service users in various settings

* Develop and complete positive risk assessments

* Research and devise Community Profiles, sourcing activities in service user's communities

* Develop and maintain partnerships with local organisations

* Building community capacity to develop more inclusive communities

* Foster the continual review of service user progress

Requirements:

* Third-level graduate in health and social care or equivalent (minimum Level 7)

* Experience working with adults with disabilities

* Design and delivery of individual programmes for people with disabilities or those who are marginalized

* Well-developed people management skills with proven experience of building, leading, and motivating a team

* Promotion and support of independent living and community integration for service users

* Development of employment and community-based opportunities

* Facilitation, implementation, and review of person-centred plans

* Managing and maintaining a focus on Health, Safety, Hygiene, and good housekeeping user's guidelines

* Development of relationships with stakeholders in the provision of health & social care services

* Awareness of budget management and cost control

Salary:

%37,138.50 - %43,028.82 per annum (pro rata), depending on experience and qualifications
