Key Responsibilities:
* Provide professional psychosocial support to patients and their families.
* Conduct assessments and deliver suitable interventions.
* Support individuals coping with illness, loss, trauma, or end-of-life care.
* Participate in discharge planning and work with community services to ensure follow-up care.
* Offer crisis support in areas such as elder abuse, homelessness, or safeguarding.
* Maintain accurate clinical records and documentation.
* Attend and contribute to multidisciplinary team meetings, family conferences, and case reviews.
Main Requirements:
* CORU-registered Social Worker
* A recognised qualification in Social Work
* Minimum of 2 years post-qualification experience in a hospital or healthcare setting
* Strong understanding of hospital discharge processes, safeguarding, and patient advocacy
* Excellent communication, organisational, and case management skills
