Key Responsibilities
* Client Assessment: Conduct comprehensive assessments of individuals and families to identify their needs, strengths, and challenges.
* Care Planning: Develop and implement individualized care or intervention plans in collaboration with clients and other professionals.
* Counselling and Support: Provide emotional support, crisis intervention, and counselling to individuals or families facing challenges such as trauma, abuse, addiction, or mental health issues.
* Resource Coordination: Connect clients with appropriate resources, including social services, healthcare providers, housing, financial aid, and employment support.
* Advocacy: Advocate on behalf of clients to ensure they receive appropriate services and support, while protecting their rights and dignity.
* Case Management: Monitor and document client progress, adjusting care plans as necessary. Maintain accurate and confidential case files.
* Collaboration: Work closely with other social workers, healthcare professionals, educators, law enforcement, and community organizations to coordinate holistic care.
* Community Outreach: Participate in outreach activities and community education programs to raise awareness of social issues and services available.
* Crisis Intervention: Respond to emergency situations and provide immediate support and guidance to individuals or families in crisis.
* Report Writing: Prepare and present reports, assessments, and case summaries for internal use and external agencies, when required.
* Professional Development: Stay updated on the latest social work practices, policies, and legislation, and attend relevant training or workshops.
Essential
? Level 7/8 bachelor’s degree in social work or related field.
? 3 years minimum experience within a similar social care setting
