Job Summary

This is a challenging opportunity for an experienced Advanced Nurse Practitioner or Trainee ANP to join our team. The successful candidate will be responsible for delivering safe, effective and person-centered care to patients in their own homes, care homes and other community settings.




Key Responsibilities:



* Delivering high-quality patient care in accordance with established policies and procedures.

* Evaluating patient needs and developing individualized treatment plans.

* Providing education to patients on health conditions, treatments options and wellness strategies.

* Maintaining accurate records of patient information including medical histories, test results and treatment outcomes.

The ideal candidate will have a Postgraduate Diploma in Advanced Practice at Masters level and be assessed as competent in this level of practice. They should also possess excellent communication skills, the ability to work autonomously within a multidisciplinary team environment; initiative sound clinical judgment abilities that enable them best serve vulnerable populations seeking refuge from life-threatening emergencies beyond ordinary hospital settings.",
