**Job Description**: As a Clinical Nurse, you will develop tailored care & service plans for clients, ensuring their needs are met and they receive the best possible support. You will work closely with clients, families, and healthcare professionals to understand their requirements and create personalized plans that meet their unique needs.
**Responsibilities**:
* Meet with clients and families to discuss their needs and develop tailored care & service plans
* Ensure regular quality assurance visits for clients
* Maintain up-to-date client records efficiently using IT systems
* Demonstrate open and effective communication with colleagues, healthcare workers, and clients and family members
* Uphold company policies, procedures, and ethical standards within the team
* Conduct Family Education sessions as needed
**Requirements**:
* Clinical Nurse NMBI Active Pin
* Full driver's license and car
* Prior experience in Home care, Nursing home or any other Social care sector would be preferred
