Role Purpose The discharge coordinator is responsible for the provision of quality service in line with standards of administration practice in a clinical area. As a key member of the frontline team, the Discharge Coordinator assists in the delivery of individualised patient care through the provision of adequate administration support. The post holder will be responsible for the effective coordination of patient discharges throughout the hospital. The position requires a collaborative and supportive approach to the development of services and structures, embracing continuous quality improvement and the implementation of changes necessary to achieve organisational objectives. Qualifications Minimum 4 years clinical experience. Relevant registration with relevant governing body Prior experience in hospital setting desirable. 1.Key Areas of Responsibility Liaise with the multi-disciplinary team (MDT) to facilitate accurate assessment of patient needs including reviewing the patients home set up to ensure appropriate provisions and care is in placeunder the auspices of what community funding/resource is available. Ensure that patients, relatives and carers are provided with information about discharge planning and that the information and timescales are fully explained and the appropriate level of support is provided with the support of the Nurses/CNMs and MDT Liaise with care/nursing homes to ensure timely assessments and completion of required documentation to facilitate discharge. Coordinate and work with the nursing team for booking transport for patient discharge Attend the daily bed management meeting with ward managers to discuss and coordinate discharge planning for all new admissions, including emergency department admissions. Liaise with nursing staff and MDT to ensure appropriate plans are in place for discharges including home care packages applications, Public Health Nurse Referrals and all other available community services as appropriate. On a daily basis maintain effective communication with the senior nurse in charge of each ward/Nurse Services Manager. To maintain accurate and concise records that provides the necessary data for the purpose of evaluating and developing the service. As an ongoing process, assesses patients, their circumstances and plans their transition from the acute hospital environment to an appropriate setting to meet the patient needs when discharged Liaise with Ward Staff to ensure the delivery of the agreed transition plan as well as the execution of an education plan as required by the patient and family. Participates with all other departments as appropriate and required to ensure the safe and efficient management of patientsdischarges in line with availability in each community health service area Promote a patient centred culture of openness and responsiveness to positive and constructivepatient feedback and ensuring patient dignity, respect and choice is maintained in their care. Liaises regularly with the Senior Medical Social Worker in relation to discharge issues, concerns and obstacles. 2.Communication Skills Communicate with the appropriate identified care agencies regarding patient discharge needs. Communicate appropriately with colleagues and other members of the health care teams. Communicate in a manner that avoids antagonism, reduces conflict and prevents undue anxiety. Recognise limitations and seek assistance where necessary. Attend and contribute to appropriate meetings. Accept constructive advice when appropriate. Report all incidents immediately, which may adversely affect a patient, colleague or the Blackrock Clinic. 3.General Expectations Support the philosophy, objectives and goals of Blackrock Clinic. Observe the appropriate lines of authority. Maintain strict confidentiality regarding all patients and the activities of Blackrock Clinic. Demonstrate flexibility by assisting in all areas of the hospital/clinic as required. The Ward/Unit Manager depending on the availability of locum cover will arrange annual leave; this usually means the holiday periods i.e. Easter, summer and Christmas. Participate in developing and providing quality care within Blackrock Clinic. Assist in orientation of new personnel. Continue to develop personal knowledge and skills and seeks outside educational opportunities. Present a professional appearance and conform to the dress, hygiene and infection control policies of the Clinic. Is always punctual on duty.
