To provide exceptional service to clients and families/significant others in accordance with best practice in case management.
This role involves working with individuals who require support in achieving their recovery goals, promoting a healthy lifestyle and empowering them to make informed decisions about their future.
Main Responsibilities:
* Engage with clients at a formal and informal level to build trusting relationships, treating them with dignity and respect at all times.
* Ensure all relevant documentation is up-to-date, accurate and compliant with project standards.
* Conduct thorough assessments of client needs, identify and develop care plans within an ongoing key working system.
* Makes referrals to internal and external services as required, including initial and comprehensive assessment, care plans and primary treatment plans.
* Facilitate group therapy sessions, psycho-educational workshops, community meetings and formal/informal gatherings in a professional and ethical manner.
* Participate in effective team meetings, attend supervision, client case reviews and be open to feedback and appraisal from colleagues and management.
* Comply with all Health and Safety regulations and standard operating procedures.
Person Specification:
* Valid Irish Work Permit for the EU and Ireland.
* Minimum of NFQ Level 7 Accredited Undergraduate Diploma in addiction studies or related field.
* TWO YEARS' EXPERIENCE WORKING WITHIN THE DRUG AND ALCOHOL FIELD IS DESIRABLE.
