Job Title: Community Care Support Officer

The purpose of this role is to provide support care in the community by assisting both the Care Coordination Team, the Scheduling Team and the Quality, Health and Safety Officer. This is an opportunity for experienced professionals to develop additional skills in relation to supervision and quality management and gain a wider experience of the workings of home support while maintaining their care skills.


Key Responsibilities:

* To undertake client reviews as agreed with the Care Coordination Team.
* To report any concerns to the Quality Health and Safety Officer/Care Manager/relevant Coordinator.
* To carry out HSW staff introductions with new clients.
* To support HSW shadowing and be a point of contact for the support of new HSW staff within working hours.
* Rapid Response - to undertake emergency cover calls including sick leave and temporarily filling new home care packages/hospital discharges within working hours.
* Support and assist with Care Team development - including conducting Spot Checks and some mentoring.
* To deliver and check Client Care books for compliance and collect/replace log sheets.
* Delivery of Care Support Plans and PPE to HSWs and service users as required.


Requirements:

* A full award QQI Level 5 or equivalent.
* A minimum of three years experience as a Home Support Worker.
* A full clean driving licence.


Benefits:

The two successful CSOs will alternate their working weeks, which will comprise of a good balance of early and late shifts and every second weekend off. The working week will consist of Monday-Friday Early Shift: 14:00-19:30 Late Shift: 16:00-21:30 Weekend Shifts: Either 08:00-20:00 or 09:00-21:00 as agreed, according to business needs. Office Shift: Three hours per week to plan visits, interact with office team and submit/file weekly paperwork. Over the two weeks 72 hours are worked = 36.5 hour average working week.


Additional Requirements:

* The successful candidate will have to have proven reliability in terms of punctuality and attendance at work.
* The successful candidate will need to have own transport and be able to travel across the whole service catchment area of Finglas, Cabra and Drumcondra as needs require.


Probationary Period:

A six-month probationary period will be part of this contract.


Salary:

€35,040
