We are looking for individuals to join our team as Rehabilitation Assistants. As a key member of our team, you will be responsible for providing support and assistance to people with acquired brain injury on all aspects of activities of daily living, enabling them to live valued lives of optimal independence and participation in community activities.
The role involves forming therapeutic relationships and rapport with individuals served, maintaining a positive constructive and professional relationship. You will also facilitate and coordinate the daily independence of individuals within the service/community by working with them and their families to formulate individual rehabilitation plans (IRPs).
Responsibilities include assisting and encouraging individuals to achieve their goals as identified in their IRPs, budgeting, money management, and disability benefits where applicable, in line with organisational policy. You will also assist individuals to improve their quality of life by developing, encouraging, and supporting them in activities of daily living, learning skills for gaining employment, and leisure activities.
You will record and document daily activities, reports, and reviews via the i Planit system and work as a member of a team, attending team meetings and reviews as required.

Key Requirements:
* A relevant Health or Social Care qualification to a minimum QQI Level 5 standard
* A full driving licence
* A person-centred approach to supporting people with an acquired brain injury
* Excellent inter-personal and communication skills
* Must be able to write and speak English to a high standard
* Good IT skills
Desirable Requirements:
* Previous residential care or community work experience
* Previous experience of working in an area of disability/illness
